Endoscopic necrosectomy through a lumen-apposing metal stent resulting in perforation: is it time to develop dedicated accessories?

چکیده
A novel lumen-apposing self-expandable metal stent (LAMS; Axios EC; Boston Scientific Corp., Marlborough, Massachusetts, USA) that is able to create a stable communicating fistulous tract has simplified the performance of endoscopic necrosectomy for walled-off pancreatic necrosis (WOPN). A 44-year-old man with a 10-cm WOPN (>50% solid component) underwent successful endoscopic ultrasound (EUS)guided drainage with placement of a LAMS (15-mm wide×10-mm long). Balloon dilation of the central part of the stent up to 18mm was performed (▶Fig. 1 a), followed by removal of necrotic debris using various Dormia baskets (▶Fig. 1b; ▶Video1) and instillation of hydrogen peroxide into the cavity. The following day, the patient developed peritoneal signs and an abdominal computed tomography (CT) scan showed perforation of the wall of the WOPN on the opposite side to the position of the stent (▶Fig. 2). He underwent successful surgery. After the videos of the proceE-Videos

A 14-year-old boy presented to our care with severe necrotizing pancreatitis secondary to a psychiatric medication. He had developed walled-off pancreas necrosis (WOPN) in the body of the pancreas that had become infected, as evidenced by air within a 10-cm collection on computed tomography (CT) scanning, fevers to 102°F, tachycardia, and leukocytosis. The CT scan showed the collection had a ma...

Necrosectomy through a metal stent is feasible [1] but can be hindered by the limited mobility of the scope tip, and accidental removal of the stent during the extraction or fragmentation of necrotic material is not uncommon [2–4]. We describe a technique for removing and replacing the same lumen-apposing stent before and after each session of necrosectomy. A 45-year-old patient with severe alc...
متن کاملNobody really knows how to perform endoscopic necrosectomy
The advent of lumen-apposing metal stents (LAMSs) as a tool in endoscopic ultrasound (EUS) has had what can only be called a transformative effect on EUS‐guided drainage of pancreatic fluid collections (PFCs) including pseudocysts and, more importantly, walled‐off pancreatic necrosis.[1-6] These procedures are being performed at more centers and in much greater numbers than ever before. Older t...
متن کاملIs there any tip that makes performing EUS-guided drainage easier and safer?
Endoscopic ultrasound (EUS)-guided drainage is evolving and no standardized technique for it has been established yet. Availability of dedicated devices and accessories also are lacking. Anderloni et al. described a new technique of stent deployment named Intra Channel Stent Release Technique (ICSRT) in EUS-guided drainage [1], which is summarized as follows: deployment of the proximal part of ...

A 32-year-old man presented to our institution with abdominal pain, anorexia, fevers, and leukocytosis from a large walled-off necrosis. Prior to this presentation he had been admitted to a local hospital 6 weeks previously with acute severe alcohol-related pancreatitis, which entailed a prolonged stay in the intensive care unit due to multiorgan failure. He was found to have an abdominal fluid...
